Preschool Online Registration

 

This page will allow you to begin the registration process for your child from our website.  If you have more than one child to register, you will need to submit addtional online forms for each of them.  By submitting this form, you will alert the preschool director that you intend to register.  However your registration is not finalized until your registration fee of $35 is paid.  Make payments out to:

Enrollment is on a "first-come, first-served" basis.  To be sure that your class is available, please register and send in your fee early. 

If you would prefer, you may register by mail or in person by printing out the Program Guide PDF that is located on this website, filling it out, and submitting it with your registration fee to the preschool.

The first time your child is picked up by any new person, we will check the driver's license in order to verify their identity.  If the driver is included in the list below, your child will be released to the driver if he/she knows your child's ID number.  If the driver is not listed below, then he/she will be required to give the preschool the child's ID number as well as to allow us to make a copy of their driver's license and signing a form which states that your child is being released to him/her.  These precautions are taken to insure the safety of all children attending our school.  Please list up to four people (in addition to the parents who are authorized to pick up your child.

Miscellaneous Release Form Items

I give Christ United Methodist Pre-School permission to take photos or videos of my child.  I understand that the photos and/or videos will NOT be released to the press without specific written consent.

 

I hereby give my written consent and permission for my child to attend any of the field trips that his/her class takes with the Christ United Methodist Pre-School during the 2008-09 school year. I realize that transportation will be provided by parents. 

 

Please include our names (parents) on my child's class listing of all the students' names, addresses and phone numbers. If you select the "No" response, your names will not be included next to your child's name. 

 

I give permission to Christ U. M. Church to use a photo of my child involved in preschool activities on their web page. I understand that no child's name or any other information regarding them will be given. 

 

We have an unlisted phone number, please do not include it on the class list.
